Local Child Safeguarding Practice Reviews and Safeguarding Adult Reviews

Local Child Safeguarding Practice Reviews (LCSPRs) and Safeguarding Adults Reviews (SARs) are a statutory requirement for Safeguarding Partnerships.  A review may be commissioned when a child or adult with needs for care and support has either died or suffered serious harm and there are concerns about how partner organisations worked together to safeguard the child or individual.  The purpose of a review is to promote the learning throughout the partnership and to improve practice.  Details of any published  SAR's or LCSPR's are available below. 

Child Death Reviews

All child deaths are reviewed with the aim to identify learning to prevent other children from dying. Child Deaths are reviewed by the Child Death Overview Panel (CDOP). This is a joint panel across Durham and Darlington Safeguarding Partnership.

Domestic Abuse Related Death Reviews

Domestic Homicide Reviews (DHRs) in England and Wales will be renamed Domestic Abuse Related Death Reviews (DARDRs) to better recognise suicides linked to domestic abuse. Domestic Homicide Reviews (DHR) were established on a statutory basis under section 9(3) of the Domestic Violence, Crime and Victims Act 2004. Responsibility for establishing a DHR is placed with the Chair of the local Community Safety Partnership (CSP) in consultation with multi agency partners.
